Scholarship Renewal Application

To renew your award, this form must be completed and delivered to the Financial Aid office of the institution you are attending, together with the following:

  1. The enclosed Financial Aid Worksheet, to be updated and completed by the Financial Aid office.
  2. A letter from you updating the Foundation on your progress and activities.
  3. An unofficial transcript of your most recent grades.
This form and the other three documents must be delivered to the Financial Aid office in sufficient time to allow that office to complete the Worksheet and mail all four documents to the Foundation on or before April 30, 2008, which is the postmark deadline.
