Maxwell Scholarship Renewal

To renew your award this document must be completed and submitted to the Foundation on or before April 30, 2016, together with the following:

1. A letter from you updating the Foundation on your campus and community activities.
2. An unofficial transcript of your most recent grades.

You must also have a Financial Aid Worksheet (in the form available at updated and completed by the Financial Aid office of the institution you are attending. Please submit a copy of The Financial Aid Worksheet to the Financial Aid office as soon as possible. This will allow that office to complete and mail the Worksheet to the Foundation.
